29 CFR  1926.501(b)(1):Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  On or about March 17, 2025, employees engaged in roofing work from the roof of a commercial building working from heights of more than 6-feet above the lower level without fall protection.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   On or about March 17, 2025, employees engaged in roofing work on a commercial building used extension ladders that did not have the side rails extended at least 3-feet above the upper landing surface being accessed.
