29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  a) On or about and times prior to, March 20, 2025, employees engaged in maintenance and servicing activities did not utilize a lockout/tagout procedure on the Adria Press Brake (S/N: 6008/11012). Employees' hand/arms were exposed to the unexpected energization of the press brake exposing employees to an amputation & crushing hazard.  b) On or about and times prior to, March 17, 2025, employees engaged in maintenance and servicing activities did not utilize a lockout/tagout procedure on the 70Ton Betenbender Press Brake (S/N: 79199). Employee's hands/arms were exposed to the unexpected energization of the press brake exposing employees to an amputation & crushing hazard.

Use of control circuitry devices (i.e., on/off switches) were utilized as a means to shut down equipment prior to performing maintenance and servicing activities.
