Instances of inadequate scaffold support included but was not limited to:  i.	i. Safway tubular welded frame scaffold #1 approximately 6 feet, 5 inches from the ground where not all scaffold poles bore on mud sills and at least one mud sill created an unstable foundation, and   ii.	Tubular welded frame scaffold #3 approximately 6 feet, 8 inches from the ground where the poles did not bear on base plates.
