29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents:  a) Scaffold on front side of building: On or about March 21, 2025, the employer exposed employees to struck-by hazards, in that protective eyewear was not provided to employees using hammer and chisel to chip bricks while engaged in masonry activities.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Cross braces were used as a means of access.  a) Scaffold: On or about March 21, 2025, The employer exposed employees to falls of 14-feet to the lower ground level, in that a ladder or other safe means of access was not provided to employees using a tubular welded masonry walk-through scaffold system engaged in masonry activities.

29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms.  a) Scaffold: On or about March 21, 2025, the employer exposed employees to fall hazards of 14 feet to a lower level, in that personal fall arrest system or guardrail systems were not provided to employees while performing masonry work on a two section high tubular welded scaffold system.
