29 CFR  1926.501(b)(10): Employees engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, were not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system:   (a)Hub City Roofing, Inc. at 1023 6th Avenue Southeast, Aberdeen, SD 57401:  On or about and at times prior to March 24, 2025, the employer did not ensure that employees were protected from falls while conducting roofing operations on a low slope commercial property. This condition exposed the employees to fall hazards greater than six feet.

29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer:  (a)Hub City Roofing, Inc. at 1023 6th Avenue Southeast, Aberdeen, SD 57401:  On or about and at times prior to March 24, 2025, the employer did not ensure a written training certification on the use of fall protection systems prior to employees being exposed to fall hazards while working on a commercial property. This condition exposed employees to fall hazards greater than six feet.

29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator was competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):   (a)Hub City Roofing, Inc. at 1023 6th Avenue Southeast, Aberdeen, SD 57401:  On or about and at times prior to March 24, 2025, the employer did not ensure that all forklift operators were competent to operate a powered industrial truck safely by a successful completion of a training program.  This condition exposed employees to struck-by hazards associated with improper forklift operations.
