29 CFR  1910.147(c)(7)(iii)(A): Retraining was not provided for authorized and affected employees when there was a change in their job assignments, a change in machines, equipment or processes that presented a new hazard, or when there was a change in the energy control procedures.  Location: a) Worksite; dumbwaiter equipment and shaft  a) On or about 3/25/2025, the employer did not provide retraining to ensure that authorized employee(s) had the appropriate knowledge and skills required for the safe application and usage of energy controls such as lockout. There was a change in machines, equipment and/or processes that presented a new hazard such as, but not limited to, entering the dumbwaiter shaft below the dumbwaiter car to repair the electric motor/cable equipment without deenergizing the system. Authorized employee(s) did not recognize the methods and means necessary for lockout/tagout when repairing and/or maintaining equipment such as, but not limited to, the dumbwaiter.
