Located at 4145 Checkerberry Court, Hilliard, OH 43026:  On or about March 26, 2025, employees were performing a shingle roof replacement on an existing residential roof without fall protection. As a result, the employees were exposed to fall hazards from approximately 12 to 24 feet.

Effective January 1, 1998, body belts are not acceptable as part of a personal fall arrest system. Note: The use of a body belt in a positioning device system is acceptable and is regulated under paragraph (e) of this section.  a.  Located at 4145 Checkerberry Court, Hilliard, OH 43026:  On or about March 26, 2025, employees were performing a shingle roof replacement on an existing residential roof with the use of unapproved climbing/rappelling harnesses as personal fall arrest systems.  The employees were exposed serious injuries resulting from a free fall from the roof edge.

Located at 4145 Checkerberry Court, Hilliard, OH 43026:  On or about March 26, 2025, employees were exposed to fall hazards when using a rope lifeline attached to ridge anchors that were not properly fastened to the roof structure. This condition exposed employees to a fall hazard of approximately 24'.

29 CFR  1926.502(d)(21): Personal fall arrest systems shall be inspected prior to each use for wear, damage and other deterioration, and defective components shall be removed from service.  a.  Located at 4145 Checkerberry Court, Hilliard, OH 43026:  On or about March 26, 2025, the employer failed to perform inspections of the fall protection systems installed on the roof to ensure that an approved personal fall arrest system was in place and utilized by each employee working from the roof. Such deficiencies included the use of unapproved climbing harnesses and improper installation of ridge anchors. These conditions exposed employees to fall hazards from approximately 12 to 24 feet.

Located at 4145 Checkerberry Court, Hilliard, OH 43026:  On or about March 26, 2025, roofing employees used a portable ladder balanced on the ridge of the lower section of roof to access the upper section of roof.  The area around the bottom of the ladder was not kept clear of supplies, trash, and debris, which exposed employees to slip, trip, and fall hazards.
