29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    (a) On or about March 25, 2025, at jobsite located at 1121 James Hopkins Rd. SW, Jacksonville, AL; employee(s) were not utilizing fall protection while replacing damaged shingles.   Exal Enrique Morales Mejia was previously cited for a violation of an equivalent Occupational Safety and Health Standard 29 CFR 1926.501(b)(11), which was contained in OSHA inspection number 1636851, citation number 1, item number 1a and was affirmed as a final order on 02-13-2023, with respect to a workplace located at 17890 SR-174, Pell City, AL 35125.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  (a) On or about March 25, 2025, at jobsite located at 1121 James Hopkins Rd. SW, Jacksonville, AL, employee(s) performing roofing activities were not trained to recognize the fall hazards.  Exal Enrique Morales Mejia was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1926.503(a)(1), which was contained in OSHA inspection number 1636851, citation number 1, item number 1b and was affirmed as a final order on 02-13-2023, with respect to a workplace located at 17890 SR-174, Pell City, AL 35125.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.
