29 CFR 1910.23(c)(6): The employer did not ensure that no ladder is moved, shifted, or extended while an employee is on it.  A)181 Herrod Boulevard. Dayton NJ 08810 - The step lock mechanism on the mobile ladder cart(s) was defective, posing a risk of accidental movement while employees were working on the ladder. This condition exposed employees to fall hazards.   Hazards observed on or about March 26, 2025.

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:  a) Location: Battery charging stations at the back of the warehouse.  A wall-mounted emergency eyewash was provided in the warehouse where electric forklifts are charged and maintained, and employees(s) are exposed to corrosive battery acid during replenishment of water and/or electrolyte to battery cells; however, the dual 946 mL squeeze bottle eyewash had an expiration date of 05/2024 and did not have sufficient capacity. This condition exposed employees to eye injuries.   **Note:  An emergency eyewash should have sufficient volume capacity for delivering at least 0.4 gallons (1.5L) of clean water per minute continuously for at least 15 minutes, as recommended in American National Standards Institute (ANSI) Standard Z358.1.  Conditions(s) were noted on or about 3/26/2025.
