East Lot - On or about March 26, 2025, an employee standing under an elevated load was fatally injured when a 20-foot truck chassis shifted and fell off the forklifts, striking the employee, resulting in fatal injuries.  The employer did not prevent employees from standing under the elevated portion of powered industrial trucks.

East Lot - On or about March 26, 2025, the employer failed to ensure that only stable or properly secured loads were handled by powered industrial vehicles.  An employee was struck-by a truck chassis' landing gear when the 20-foot chassis shifted and fell from two forklifts, resulting in fatal injuries.

29 CFR 1910.178(o)(2): Load(s) were being handled which exceeded the rated capacity of the industrial truck(s):  a.) East Lot - On or about March 26, 2025, the employer did not ensure that Toyota Forklift, Model 42-6FGU25, Serial No. 62536 and Toyota Forklift, Model 8FGCU25, Serial No. 13955, were not prevented from being used to handle loads exceeding the rated capacity.  Forklifts were used to lift a 20-foot truck chassis weighing approximately 6,700 lbs. which exceeds the lifts' load capacities.
