NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR  1904.40(a):The employer did not provide an authorized government representative the OSHA 300 injury and illness records within the four business hours.  a) Justice Grown/Oakland Management LLC, 1474 Prospect Street Ewing, NJ 08368: On or about 3/28/2025,  at approximately 8am the employer failed to provide their 2022, 2023, 2024, and 2025 OSHA 300 and OSHA 300a forms or equivalent to the Compliance Safety and Health Officer within 4 hours. The establishment employed 76 employees and was classified as NAICS 111419 during the calendar year 2025.

29 CFR 1910.134(c)(2)(i): Respirator users were not provided with the information contained in Appendix D to 29 CFR 1910.134 when the employer determined that any voluntary respirator use was permissible:  a) Justice Grown/Oakland Management LLC, 1474 Prospect Street Ewing, NJ 08368: On or about 3/27/2025, employees provided with elastomeric half face respirators, N95 respirators and PAPRs for voluntary use were not provided with the information contained in Appendix D.   NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
