During the performance of duties in aisles 71 and 73 in The Cooler, employees were required to walk on floor surface areas on which ice had accumulated.  This condition exposed employees to slip and fall hazards.

29 CFR  1910.22(d)(2): The employer did not ensure that hazardous conditions on walking-working surfaces are corrected or repaired before an employee uses the walking-working surface again. The employer did not ensure that if the correction or repair cannot be made immediately, the hazard was guarded to prevent employees from using the walking-working surface until the hazard is corrected or repaired:   a)  On or about March 26, 2025, and at times prior, the employer, DFA Dairy Brands Distributing West, LLC DBA Meadow Gold Dairy, did not ensure employees working in the Cooler area were protected from falling ice debris.  During the performance of duties in aisles 71 and 73 in the Cooler, employees were required to walk under cooling system components attached to the ceiling and on which hanging ice had accumulated.  Before requiring employees to work in these areas, the employer did not correct the hazardous condition or guard the area to prevent employee access.  This condition exposed employees overhead, struck-by hazards capable of causing severe injury.
