29 CFR  1910.1200(e)(1):Employer had not developed or implemented a written hazard communication program that included the requirements outlined in 29 CFR 1910.1200(e)(1)(i) and (e)(1)(ii):   At a worksite located at Lot 711 Stayton Ferry Loop, Columbia, MO 65201, employees were exposed to health hazards while using chemicals.  The employer exposed the employees to these hazards by allowing them to utilize chemicals that included, but not limited to "4000 subfloor construction adhesive" without first being instructed on the hazards associated with chemical use or methods to properly handle them.

29 CFR  1926.403(b)(2):Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification:  At a worksite located at Lot 711 Stayton Ferry Loop, Columbia, MO 65201, employees were exposed to electrical hazards that include electrical shock, electrical burns and electrical fire.  The employer exposed the employees to these hazards by allowing them to utilize multiple extension cords connected to each other to provide electrical power to electrical devices used at the worksite.

The employer exposed the employees to these hazards by allowing them to perform framing work at heights greater than 6 feet without the use of fall protection.

The employer exposed the employees to these hazards by allowing them to work above uncapped rebar that protruded vertically from the foundation of the structure.
