On top of a residential structure located at 4515 Woodsong Loop E.  Jacksonville, FL: On or about April 2, 2025, the employer exposed employees to fall hazards, in that fall protection was not utilized by employees performing re-roofing operations.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:   a. Positioned in the front  of a single-family structure located at 4515 Woodsong Loop E.,  Jacksonville, FL: On or about April 2, 2025, the employer exposed employees to fall hazards, in that a 10-foot Gorilla extension  ladder was used to access the top of the structure and did not extend three feet above the eave as required.
