29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet above the upper landing surface to which the ladder was used to gain access:   a) 608 North Street, Milford, DE 19963: On or about April 2, 2025, employees used ladders that did not extend at least 3 feet above the upper landing to access the upper levels of a roof, approximately 11 feet above the ground, exposing them to falls.

29 CFR  1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by a guardrail system, safety net system, or personal fall arrest system, or an alternative fall protection measure that meets a provision described in paragraph (b) of this section:  a) 608 North Street, Milford, DE 19963: On or about April 2, 2025, employees were seen working from elevated surfaces approximately 11 feet above grade without fall protection while removing the roofing material of an existing single family, single story home.  Robert Grant, Inc. dba Grant & Sons Roofing & Siding was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1596528, citation number 1, item number 1 and was affirmed as a final order on January 2, 2023, with respect to a workplace located at 705 South Dupont Highway, Milford, DE.
