29 CFR  1910.106(d)(5)(i): "Egress." flammable liquids, including stock for sale, were stored in a location that limited the use of exits, stairways, or areas normally used for the safe egress of people.   At a worksite located at 505 North Glenstone, Springfield, MO 65802:  Employees were exposed to fire hazards. This occurred on or about April 3, 2025, and at times prior to when employees were working in an area known as the vinyl room where flammable liquids were stored in the form of  Isopropyl Alcohol 99%. The employer failed to keep exit routes free of highly flammable liquids that would be used by employee as a safe egress.

29 CFR  1910.107(b)(9): A clear space of not less than 3 feet on all sides was not kept free from storage or combustible construction:  At a worksite located at 505 North Glenstone, Springfield, MO 65802:  Employees were exposed to fire hazards. This occurred on or about  April 3, 2025, and at times prior to when employees were working in paint booth(s) where combustible materials were stored within 3 feet or less of the paint booth perimeter. This included, but was not limited to cardboard materials, wooden cabinets, and flammable liquid containers.

29 CFR  1910.1052(d)(2): The employer did not perform initial monitoring to determine each employee's exposure to methylene chloride:   At a worksite located at 505 North Glenstone, Springfield, MO 65802:   Employees were exposed to toxic or hazardous substances in the form of methylene chloride. This was demonstrated on or about April 3, 2025, and at times prior to, when employees were applying Weld-On #3 and Weld-On #16 that contained methylene chloride. The employer failed to ensure employees were not exposed to concentrations of methylene chloride over 25 parts per million over an 8-hour time-weighted average by performing initial monitoring.

29 CFR  1910.1052(l)(1): The employer did not provide information and training for each affected employee prior to or at the time of initial assignment to a job involving potential exposure to methylene chloride:  At a worksite located at 505 North Glenstone, Springfield, MO 65802:   Employees were exposed to toxic or hazardous substances in the form of methylene chloride. This was demonstrated on or about April 3, 2025, and at times prior to, when employees were applying Weld-On #3 and Weld-On #16 that contained methylene chloride. The employer failed to provide information and training to employees who used the chemical containing methylene chloride regarding their exposure hazards prior to or at the time of the initial assignment.
