On or about March 28, 2025, at field jobsite, employee(s) raised the vehicle rig boom within 10 feet of the overhead power lines while preparing for water well maintenance.

Column G through M on the OSHA 300 log for the year 2024 was not totaled to reflect the number of injuries that were recorded for the year.  b) On or about April 3, 2025, and times prior to, and thereafter, the employer did not verify that the entries on the OSHA 300 log(s) for the year 2023 were complete and accurate. Column G, I, J, L and M on the OSHA 300 log for the year 2023 was not totaled to reflect the number of injuries that were recorded for the year.
