29 CFR  1926.403(b)(1)(iii):  Examination.  The employer shall ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees. Safety of equipment shall be determined based on electrical insulation.  On or about April 8, 2025, at a worksite located at W5353 Roundstone Place Lot 40 Harrison Heights 2 in Menasha, Wisconsin, an employee was exposed to a potential electrical hazard using a damaged extension cord to power a circular saw. The insulation jacket was broken at the female end.

29 CFR  1926.451(e)(1):  When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface shall be used.  On or about April 8, 2025, at a worksite located at W5353 Roundstone Place Lot 40 Harrison Heights 2 in Menasha, Wisconsin, employees were exposed to fall hazards 11 feet above surface level while accessing and egressing a ladder jack scaffold platform without proper means of access.

29 CFR  1926.452(k)(4):  Ladders used to support ladder jacks shall be placed, fastened, or equipped with devices to prevent slipping.   a)  On or about April 8, 2025, at a worksite located at W5353 Roundstone Place Lot 40 Harrison Heights 2 in Menasha, Wisconsin, employees were exposed to a fall hazard 11 feet above surface level working from a ladder jack scaffold platform without installing the jack clamps to tie the supporting ladders together as a system.  b)  On or about April 8, 2025, at a worksite located at W5353 Roundstone Place Lot 40 Harrison Heights 2 in Menasha, Wisconsin, an employee was exposed to a fall hazard 14 feet above surface level working from a ladder jack scaffold platform without installing the jack clamps to tie the supporting ladders together as a system.  c)  On or about April 8, 2025, at a worksite located at W5353 Roundstone Place Lot 40 Harrison Heights 2 in Menasha, Wisconsin, employees were exposed to a potential fall hazard of 11 feet working from a ladder jack scaffold platform supported by extension ladders on top of soil without using the toe of the safety feet to prevent the ladder from slipping.

29 CFR  1926.454(b)(2):  The employer shall have each employee who is involved in erecting, disassembling, moving, operating, repairing, maintaining, or inspecting a scaffold trained by a competent person to recognize any hazards associated with the work in question. The training shall include the correct procedures for erecting, disassembling, moving, operating, repairing, inspecting, and maintaining the type of scaffold in question.  On or about April 8, 2025, at a worksite located at W5353 Roundstone Place Lot 40 Harrison Heights 2 in Menasha, Wisconsin, the employer did not provide required scaffold training for erectors to be able to identify hazards, such as fall from elevations on ladder jack scaffolds, installing of jack clamps, the correct access/egress equipment, and installing support ladder safety feet.

The training shall include the nature of any electrical hazards, fall hazards, and falling object hazards in the work area.  On or about April 8, 2025, at a worksite located at W5353 Roundstone Place Lot 40 Harrison Heights 2 in Menasha, Wisconsin, the employer did not provide required scaffold use safety training for employees who were working from ladder jack scaffold platforms at heights greater than 10 feet.
