29 CFR  1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:   (a) On or about April 08, 2025, at the jobsite located at 16700 US Highway 411, Odenville, AL:  the employer exposed employees to fall hazards, in that fall protection was not utilized by an employee in an aerial lift  while installing panel to the side of a building.

29 CFR  1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:   (a) On or about April 08, 2025, at the jobsite located at 16700 US Highway 411, Odenville, AL:  the employer exposed an employee to a fall hazard, in that fall protection was not utilized by the employee while performing roofing work.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  (a) On or about April 08, 2025, at the jobsite located at 16700 US Highway 411, Odenville, AL: the employer exposed employees to fall hazards, in that fall protection training was not provided to employees while performing roofing work.
