I.	Lathe Department: 1.	The rotating Clausing chuck was unguarded.  II.	Stock area/cut off area: 1.	The left and right unused portions of the Cosen horizontal bandsaw blade were unguarded. 2.	The left unused portion of the Hem horizontal bandsaw blade was unguarded.  III.	Tool Room: 1.	The rotating Wasino chuck was unguarded.
