A stairway, ladder, ramp or other safe means of egress shall be located in trench excavations that are 4 feet (1.22 m) or more in depth so as to require no more than 25 feet (7.62 m) of lateral travel for employees. At the residential worksite located at 1334 SW 14th Street Terrace in Blue Springs, Missouri: On or about April 8, 2025, employees engaged in the installation of piers for the foundation of a residential structure, were exposed to cave-ins and falling dirt, when the employer failed to ensure that a mean of egress was provided for employees working inside the trench that measured approximately seven feet in depth.

The excavation was seven feet deep with soil type cohesive type B.
