1926.20(b)(2) The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s)  a)  On or about April 7, 2025, on the east side of the drawbridge # 117: subcontractor employees were exposed to a 12 foot fall hazard while working on the bridge, in that the employer did not ensure that each employee was protected from falling through an access panel leading to the mechanical room.
