29 CFR  1910.147(d)(2): The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment.  a. On or about April 3, 2025, at the manufacturing area: employees were exposed to amputation hazards, in that, lock out and tag out (LOTO) was not performed prior to accessing an unguarded section of the Merco machine to work on a drum.  Mauser Packaging Solutions was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.147(d), which was contained in OSHA inspection number 1634694, citation number 1, item number 1 and was affirmed as a final order on 04-15-2024 , with respect to a workplace located at 4400 West 35th Street, Chicago, IL.

29 CFR  1910.212(a)(3)(ii):The  point of operation of machines whose operation exposed an employee to injury, was not guarded:  a. On or about  April 3, 2025, in the manufacturing area: employees were exposed to amputation hazards, in that, a guard was not installed on the back side of the Merco machine which is frequently transited by employees using the machine.  Mauser Packaging Solutions was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.212(a)(3)(ii), which was contained in OSHA inspection number 1604973, citation number 1, item number 3, group a, and was affirmed as a final order on 11-16-2023, with respect to a workplace located at 1385 Blatt Blvd, Columbus, OH.  Mauser Packaging Solutions was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.212(a)(3)(ii), which was contained in OSHA inspection number1529124, citation number1, item number 1 and was affirmed as a final order on 10-26-2021, with respect to a workplace located at 903 N Kilpatrick Ave, Chicago, IL.
