29 CFR 1926.502(d)(1): Connectors shall be drop forged, pressed or formed steel, or made of equivalent materials. On April 11, 2025, at jobsite location 231 - 235 Lorraine Cir., Bloomingdale, IL 60108, the employer did not ensure that the lanyard connectors and anchorage points were made of steel hardware (e.g. carabiner, hook, ring, etc.) approved for application with personal fall arrest systems. Two employees engaged in residential re-roofing of a two-story townhouse were exposed to falls of at least 10 feet to ground below due to unsafe lanyard connections. One employee's lanyard was looped around the other employee's lanyard which was also looped around a connection at the ridge. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1926.502(d)(15): Anchorages used for attachment of personal fall arrest equipment shall be independent of any anchorage being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee attached, or shall be designed, installed, and used as follows: (i) as part of a complete personal fall arrest system which maintains a safety factor of at least two; and (ii) under the supervision of a qualified person. a) On April 11, 2025, at 231 - 235 Lorraine Cir., Bloomingdale, IL 60108 job site, the employer did not ensure that the anchorage used by employees as an attachment point for their personal fall arrest systems was capable of supporting at least 5,000 pounds (22.2kN) of force per employee attached to it. Two employees engaged in residential re-roofing of a two-story townhouse had their lanyards connected to a single roof temper anchor designed for one person use only. Employees were exposed to falls of at least 10 feet to ground below due to unsafe use of an anchor. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

Employees were exposed to falls of at least 10 feet to ground below when using front straps of their body harnesses as attachment points for their lanyards. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1926.502(d)(21): Personal fall arrest systems shall be inspected prior to each use for wear, damage and other deterioration, and defective components shall be removed from service. a) On April 11, 2025, at 231 - 235 Lorraine Cir., Bloomingdale, IL 60108 job site, the employer did not ensure that employees engaged in residential re-roofing of a two-story townhouse had their personal fall arrest systems inspected for wear, damages, or other deterioration prior to each use. An employee working on the roof had his body harness connected to a damaged lanyard with separating rope strands and was exposed to an approximately 10 foot fall to ground below. There is no abatement certification or documentation required for this item.
