29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:  a) New commercial construction located at 5090 Hammond Industrial Drive, Cumming, GA 30041, Roof/Aerial Lift: On or about April 12, 2025, the employer exposed employees to ejection/fall hazards of over 18-feet to the lower ground level, in that fall arrest systems were not provided to employees using the aerial lift to work from and transport employees to the roof.

29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems.  a) New commercial construction located at 5090 Hammond Industrial Drive, Cumming, Georgia 30041, Roof: On or about April 12, 2025, employees performing roofing and gutter installation work were exposed to fall hazards while working at heights of approximately 18 feet above ground level. The employees were not provided with, nor protected by, any of the required fall protection systems.

29 CFR 1926.602(d): The employer did not certify that each operator had been trained and evaluated as required by paragraph 29 CFR 1910.178(l):  a) New commercial construction located at 5090 Hammond Industrial Drive, Cumming, GA 30041: On or about April 12, 2025, the employer did not ensure that employees operating powered industrial trucks at the job site had received the required training and evaluation. As a result, employees were exposed to struck-by hazards due to equipment being operated by untrained personnel.
