29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying objects while using a nail gun.  a) Pool house located at 4739 Wilderness Trail, Flowery Branch, GA 30542, Roof:  On or about 4/15/2025, the employer exposed employees to struck-by hazards while operating a pneumatic roofing nail guns, in that protective eyewear was not provided while operating these tools engaged in roofing activities.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toe boards, safety net systems, or personal fall arrest systems:   a) Pool house located at 4739 Wilderness Trail, Flowery Branch, GA 30542, Roof: On or about 04/15/2025, the employer exposed employees to fall hazards of 10-feet to the lower ground level, in that an adequate fall protection system was not used while installing shingles on a roof with a pitch of 10/12.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  a) Pool house located at 4739 Wilderness Trail, Flowery Branch, GA 30542, Roof:  On or about 4/15/2025, the employer exposed employees to fall hazards greater than 10 feet, in that the ladder used to access the roof of the structurer was not extended at least three feet above the upper landing surface.
