29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:    a) New residential construction located on 1720 Goodwin Drive, Hampton, GA 30228, Roof: On or about April 15, 2025, the employer exposed employees to falls of  19-feet, 11-inches to the lower ground level, in that  complete and adequate fall protection systems were not provided to employees  engaged in roof activities.

29 CFR 1926.1053(b)(5)(i): Non-self-supporting ladders were not used at an angle such than the horizontal distance from the top support to the foot of the ladder way approximately one-quarter of the working length of the ladder:  a) New residential construction located on 1720 Goodwin Drive, Hampton, GA 30228, Front side of structure to the roof: On or about April 15, 2025, the employer exposed employees to a fall hazard of approximately 19 feet, 11 inches to the ground below, in that a portable extension ladder used to access the roof was set at a shallow angle of approximately 51.2 degrees. This angle was less steep than the recommended 75.5 degrees, reducing ladder stability and increasing the risk of backward sliding or structural failure during use.

29 CFR 1926.552(a)(4): Hoisting ropes were not installed in accordance with the wire rope manufacturers' recommendations.   a) New residential construction located on 1720 Goodwin Drive, Hampton, GA 30228, Left side of the structure: On or about April 15, 2025, On or about April 15, 2025, the employer exposed employees to struck-by hazards in that a material hoist was equipped with a nylon rope in place of the manufacturer-recommended wire rope. The substitution of incompatible hoisting material compromised the structural integrity and safe operation of the hoist, increasing the risk of rope failure and falling materials during lifting operations.
