29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms before being released for use by employees other than erection/dismantling crews: a) New apartment construction located at 1400 Jr. Grant Blvd., McDonough, GA 30253, Tubular welded scaffold system: On or about April 15,2025, the employer exposed employees to falls of 19-feet, 10-inches to the lower level, in that mid and top end rails were not installed on the scaffold working platform for employees engaged in masonry activities.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502: a) New apartment construction located at 1400 Jr. Grant Blvd., McDonough, GA 30253, Roof: On or about April 15, 2025, the employer exposed employees to fall hazards of 12-feet, 6-inches to the lower ground level, in that a fall protection system was not provided to employees engaged in masonry activities using the 4/12 pitched roof as a walking/working surface.
