29 CFR 1910.95(g)(6): At least annually after obtaining the baseline audiogram, the employer did not obtain a new audiogram for each employee exposed at or above an 8-hour time-weighted average of 85 decibels:  On or about April 16, 2025 and at times thereafter, the employer did not obtain annual audiograms for all employees exposed to noise levels above an 8-hour time weighted average of 85 dBA while working in building 2 in the press area. Noise dosimetry showed:     An employee operating the Cincinnati Press ID16 was exposed to noise at 107.8% of the permissible daily dose as measured over 381 minutes of sampling. This dose is equivalent to an 8-hour TWA exposure of 90.5 dBA accounting for the period of 99 minutes unmonitored.  Failure to perform annual audiograms can lead to an inability to identify employee hearing loss.

29 CFR 1910.95(k)(2): The training program was not repeated annually for each employee included in the hearing conservation program:  On or about April 16, 2025 and at times thereafter, the employer did not ensure all employees exposed to noise levels exceeding the action level of 85 decibels while operating the Cincinnati Press ID 16 received training annually on the effects of noise, the purpose of hearing protectors and how to select, fit , use and care for the hearing protectors, the purpose of audiometric testing and an explanation of audiometric test procedures.  Failure to train employees annually can lead to deficiencies in the hearing conservation program and contribute to employee hearing loss.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):  On or about May 9, 2025, the employer did not ensure that the company's energy control procedures were utilized to control hazardous mechanical movement of the robotic welders and the ferris wheel positioner, when employees, not during set-up or repair, changed out weld tips on the welders, exposing employees to struck by hazards.

29 CFR  1910.1200(f)(6)(i): Except as provided in 29 CFR 1910.1200(f)(7) and 29 CFR 1910.1200(f)(8), the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the information required by 29 CFR 1910.1200(f)(1)(i) through 29 CFR 1910.1200(f)(1)(v).  On or about May 9, 2025 and at times thereafter, the employer did not ensure that containers storing hazardous chemicals such as, but not limited to triethanolamine, diethylene glycol, and phosphoric acid which are included in HDP 970-40 lubricant were labeled and marked with the product identifier, signal works, hazard statements, pictograms, precautionary statements, and the chemical manufacturer's name, address, and telephone number in that spray bottles located by presses throughout the facility were labeled on with an identifier of metal mate or vanishing oil.  Failure to properly label containers can lead to employee exposure to chemical and physical hazards.
