29 CFR 1910.95(g)(6): At least annually after obtaining the baseline audiogram, the employer did not obtain a new audiogram for each employee exposed at or above an 8-hour time-weighted average of 85 decibels:  On or about April 16, 2025 and at times thereafter, the employer did not obtain annual audiograms for all employees exposed to noise levels above an 8-hour time weighted average of 85 dBA while working in building 2 in the press area. Noise dosimetry showed:     An employee operating the Cincinnati Press ID16 was exposed to noise at 107.8% of the permissible daily dose as measured over 381 minutes of sampling. This dose is equivalent to an 8-hour TWA exposure of 90.5 dBA accounting for the period of 99 minutes unmonitored.  Failure to perform annual audiograms can lead to an inability to identify employee hearing loss.
