29 CFR 1926.25(a): During the course of construction, alteration, or repairs, debris was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures: (a) Front Entry - On or about April 14, 2025, carpenters and framers were being exposed to slip, trip, fall and puncture hazards from protruding nails where debris is not being removed from the work areas in a timely manner. (b) Rear Entry _ On or about April 14, 2025, carpenters and framers were being exposed to slip, trip, fall and puncture hazards from protruding nails where debris is not being removed from the work areas in a timely manner. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification. This documentation may include but is not limited to evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a) Roof top - On or about April 14, 2025, at a worksite located at 1341 PA Avenue, SE Washington, DC, employees were performing framing work without fall protection exposing them to a fall hazard of approximately 45 feet. b) Rear 2nd, 3rd and 4th floors, Front 2nd floor - On or about April 14, 2025, at a worksite located at 1341 PA Avenue, SE Washington, DC, employees while performing framing work of a multi-level residential home near unguarded windows were exposed to a fall hazard averaging approximately 10 to 30-feet. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification. (b) Rear 2nd Floor Wall Opening - On or about April 24, 2025, carpenter workers were observed using a job made ladder, that extended approximately 1 foot above the upper landing to access the upper floors exposing them to a fall of approximately of 11 feet. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification.
