29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) 2230 North 5th Street, Reading PA - On or about April 16, 2025, an employee performing commercial siding work was exposed to a fall hazard of approximately 20 feet when the ladder was not being extended 3 feet above the landing nor secured to a rigid support with a grasping device.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.
