(a) On or about April 17, 2025, at W256 N6684 Beaver Creek Lane, Sussex, WI 53089, the employer did not ensure that employees working from a JLG 6042 SkyTrak Telehandler lift at an approximate height of 18ft were protected from falls to the lower level with fall protection.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).  (a) On or about April 17, 2025, at W256 N6684 Beaver Creek Lane, Sussex, WI 53089, employees installing sheathing on a new residential home with a 9:12 pitch were not using fall protection. This exposed employees to a fall hazard of approximately 21 feet from the roof eve to the ground below on the front (north) side of the home.

(a) On or about April 17, 2025, at W256 N6684 Beaver Creek Lane, Sussex, WI 53089, the employer did not verify fall protection training by preparing a written certification record for employees tasked with framing a new residential home.
