29 CFR  1926.20(b)(2):The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person.  LOCATION: The worksite at 93 Bennington St., Revere  a) Two lanyards were connected together and to the D-ring on the employee's harness in stead of a vertical lifeline.  b) The Personal Fall Protection System, as it was being used, would not have been capable of supporting at least 5,000 pounds (22.2 kN) per employee.  c) The anchorages used for attachment of personal fall arrest systems were not designed, installed and used under the supervision of a qualified person.  d) Personal fall arrest systems were not inspected prior to each use for wear, damage, and other deterioration, and a visibly defective lanyard was not removed from service.  e) A worker was observed standing on the mid-rail of a Mobile Elevated Work Platform (MEWP) without a restraining belt or other personal fall arrest system in place.  On or about  March 20, 2025, the employer exposed their workers to numerous hazards when no competent person was on site to prevent unsafe practices as part of a safety program that provided for frequent and regular inspections.
