Employee was exposed to a fall of approximately 10 ft while working at the 6:12 pitch roof without a fall protection system.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about April 22nd, 2025 Location: 73 Salem Rd, White Plains, NY 10603  a) Employees were exposed to approx. 8 ft fall while accessing the roof with an extension ladder that did not extend 3 ft above the upper transition point. The ladder did not extend above the landing at all.
