29 CFR  1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury:      a) On the Roof/New residential construction home located at 1799 Dawn Rd Conyers GA 30013 - On or about 04/22/2025, the employer exposed employees to struck-by hazards while operating pneumatic nail guns, in that protective eyewear was not provided to employees engaged in roofing activities operating these tools at 130 PSI.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a) On the roof/New residential  located at 1799 Dawn Rd Conyers, GA 30013, roof - On or about 04/22/2025, the employer exposed employees to fall hazards of 9-feet to 3-inches to the lower level, in that fall protection systems were not provided and used by employees engaged in roofing activities on a 4/12 pitched roof.
