29 CFR  1926.102(a)(1):The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  On or about April 23, 2025 at 4388 Fulton Road, Brooklyn OH 44146, the employer failed to ensure employees operating a pneumatic nail gun wore proper eye protection in that no eye protection was worn.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  On or about April 23, 2025, during the installation of roofing material at 4388 Fulton Road, Brooklyn, OH 44146, the employer failed to ensure employees working on a single family 1.5 story home with an 8:12 pitch roof was protected from a fall of approximately 9 feet in that no personal fall protective systems/devices were being utilized.
