29 CFR  1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:  (a)Storage room- 17 New Jersey 18 New Brunswick, NJ 08901  Suitable facilities were not provided within the work area for immediate emergency flushing of both eyes of employee(s) who change and use soaps such as LO PH soap, T&E Wheel & Rim, and Yellow LO PH Polish, which are labeled as corrosive.  **Note: The eyewash station provided was not of necessary volume capacity for delivering not less than 0.4 gallons (1.5L) of clean water per minute continuously for at least 15 minutes, as recommended in American National Standards Institute (ANSI) Standard Z358.1.  Condition(s) were noted on or about April 24, 2025

29 CFR  1904.39(a)(1):The employer did not report within 8-hours the death of an employee resulting from a work-related incident.  a) 17 New Jersey 18 New Brunswick, NJ 08901  On April 22, 2025, an employee of Trax Car Wash was vacuuming a car in the tunnel of the car wash when he was struck by a vehicle and died. The employer was aware of the employee's passing on the same day, but did not report the death to OSHA. OSHA became aware of the workplace fatality from a news clip (New Jersey News 12) on April 24, 2025.  incident occurred on about April 22, 2025.

29 CFR  1910.1200(e)(1)(i):The employer did not compile a list of the hazardous chemicals known to be present using a product identifier that was referenced on the appropriate safety data sheet.  a) 17 New Jersey 18 New Brunswick, NJ 08901  Trax Car Wash's hazard communication program did not have a list of hazardous materials known to be located and used at the workplace, including, but not limited to, LO PH soap, T&E Wheel & Rim, and Yellow LO PH Polish, which are labeled as corrosive.  Condition(s) were noted on or about April 24, 2025.
