29 CFR 1926.501(b)(11): "Steep roofs." Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems. a. Located at 2890 Airport Drive, Columbus Ohio, 43219: On or about April 24, 2025, employees were observed walking and working on the steeply pitched roof (between 5:12 and 12:12) of a three-story hotel, at heights of approximately 30 feet or more, without proper fall protection systems in place. This exposed them to significant fall hazards.

29 CFR 1926.1053(b)(1):When portable ladders are used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension is not possible because of the ladder's length, then the ladder shall be secured at its top to a rigid support that will not deflect, and a grasping device, such as a grabrail, shall be provided to assist employees in mounting and dismounting the ladder. In no case shall the extension be such that ladder deflection under a load would, by itself, cause the ladder to slip off its support. a. Located at 2890 Airport Drive, Columbus Ohio, 43219: On or about April 24, 2025, employees were observed using a single extension ladder to access the roof of a three-story hotel without ensuring that the ladder extended at least three feet above the roofline or proper stabilization to prevent deflection or movement. This exposed the employees falls of up to 30 feet to the ground below.
