29 CFR  1926.102(a)(1): The employer did not ensure employees used eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  a. On or about April 24, 2025, at the jobsite located at 15826 Rob Roy Dr. Oak Forest, IL 60452, an employee was exposed to eye injury while operating a pneumatic nail gun without wearing eye protection while installing roof shingles.  New Era Solutions Inc. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1669516, citation number 1, item number 1 and was affirmed as a final order on January 8, 2024, with respect to a workplace located at 208 1st St Libertyville, IL 60048.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a. On or about April 24, 2025, at the above addressed jobsite, employees performing residential roofing operations, a maximum nineteen (19') feet seven (7") inches above the ground, were not protected from falls, thereby exposing employees to fall hazards.  b. On or about April 24, 2025, at the above addressed jobsite, employees performing residential roofing operations, a maximum ten (10') feet six (6") inches above the ground, were not protected from falls, thereby exposing employees to fall hazards.  New Era Solutions Inc. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1669516, citation number 1, item number 2 and was affirmed as a final order on January 8, 2024, with respect to a workplace located at 208 1st St Libertyville, IL 60048.

29 CFR 1926.502(d)(17): The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head.  a. On or about April 24, 2025, employees conducting roofing operations were working 19 feet, 7 inches above the ground.  The employees were exposed to injuries due to the arresting force of the harness being located at the wears pelvis instead of the wears back near shoulder height as required by the standard.
