(a) Fortune Renovations, LLC failed to document abatement within 10 calendar days after the abatement date that each of the following cited violations had been corrected:   Citation 1, Item 1, a serious violation of 29 CFR 1926.501(b)(13) with an abatement date of September 26, 2025.    Citation 1, Item 2 a serious violation of 29 CFR 1926.1053(b)(1) with an abatement date of September 26 2025.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  (a) On or about April 25, 2025 at the jobsite located at 2202 E. Woodstock Place, Milwaukee, WI 53202, the employer did not ensure that employees working on a two-story residential flat roof without any fall protection in place were protected from fall hazard, exposing employees to a fall hazard of approximately 13 feet to the ground.  (b) On or about April 25, 2025 at the jobsite located at 2202 E. Woodstock Place, Milwaukee, WI 53202, the employer did not ensure that employees working on a two-story residential gable roof without any fall protection in place were protected from fall hazard, exposing employees to a fall hazard of approximately 16 feet to the ground.

29 CFR  1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:   (a) On or about April 25, 2025 at the jobsite located at 2202 E. Woodstock Place, Milwaukee, WI 53202, the employer did not ensure employee conducting work on the flat roof of a residential home were provided with training on how to minimize or eliminate fall hazards by a competent person. This exposed employees to a fall hazard of approximately 13 feet.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a) On or about April 25, 2025 at the jobsite located at 2202 E. Woodstock Place, Milwaukee, WI 53202, the employer did not ensure the extension ladder used by employees to access and egress the residential gable was extended at least three feet above the upper landing. Exposing employees to a fall hazard of approximately 16 feet to the ground below.  (b) On or about April 25, 2025 at the jobsite located at 2202 E. Woodstock Place, Milwaukee, WI 53202, the employer did not ensure the extension ladder used by employees to access and egress the residential flat roof was extended at least three feet above the upper landing. Exposing employees to a fall hazard of approximately 13 feet to the ground below.  (c) On or about April 25, 2025 at the jobsite located at 2202 E. Woodstock Place, Milwaukee, WI 53202, the employer did not ensure the extension ladder used by employees to access and egress the residential roof near the dormer was extended at least three feet above the upper landing. Exposing employees to a fall hazard of approximately 13 feet to the ground below.
