29 CFR 1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a) On or about April 25, 2025 at the jobsite located at 2202 E. Woodstock Place, Milwaukee, WI 53202, the employer did not ensure that employees working on a two-story residential flat roof without any fall protection in place were protected from fall hazard, exposing employees to a fall hazard of approximately 13 feet to the ground. (b) On or about April 25, 2025 at the jobsite located at 2202 E. Woodstock Place, Milwaukee, WI 53202, the employer did not ensure that employees working on a two-story residential gable roof without any fall protection in place were protected from fall hazard, exposing employees to a fall hazard of approximately 16 feet to the ground.

Woodstock Place, Milwaukee, WI 53202, the employer did not ensure employee conducting work on the flat roof of a residential home were provided with training on how to minimize or eliminate fall hazards by a competent person. This exposed employees to a fall hazard of approximately 13 feet.

Woodstock Place, Milwaukee, WI 53202, the employer did not ensure the extension ladder used by employees to access and egress the residential gable was extended at least three feet above the upper landing. Exposing employees to a fall hazard of approximately 16 feet to the ground below. (b) On or about April 25, 2025 at the jobsite located at 2202 E. Woodstock Place, Milwaukee, WI 53202, the employer did not ensure the extension ladder used by employees to access and egress the residential flat roof was extended at least three feet above the upper landing. Exposing employees to a fall hazard of approximately 13 feet to the ground below. (c) On or about April 25, 2025 at the jobsite located at 2202 E. Woodstock Place, Milwaukee, WI 53202, the employer did not ensure the extension ladder used by employees to access and egress the residential roof near the dormer was extended at least three feet above the upper landing. Exposing employees to a fall hazard of approximately 13 feet to the ground below.
