29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about April  28, 2025, employees were exposed to a fall hazard of 11.6  feet while removing shingles from a residential room located at 4320 Tara Way, MEDINA, OH 44256. Employees were using  extension and multi ladders to access the roof, and the employer did not ensure the ladder extended at least 03 feet above the upper landing surface.
