29 CFR  1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  (a) Top of house - On or about April 30, 2025, the employer had his employees apply roof nail strips to the steep roof over 30 feet high without any fall protection, exposing employees to fall hazards.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a) East side of roof - On or about April 30, 2025, the employer had his employees use the aluminum extension ladder to access the roof that did not extend 3 feet above the roof's edge, exposing employees to fall hazards.
