29 CFR  1926.1060(a):The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would train each employee in the procedures to be followed to minimize hazards related to ladders and stairways:  a) 31 Tebco Tr., Long Beach, NJ: An employee was injured from a fall from a ladder of approximately thirty-five feet to the ground below. The employee had climb the ladder to repair a portion of a third story window frame. The employee climbed thru the window and back out again when the ladder lock hooks became unlocked causing the ladder and employee to come sliding down. Observed on or about 04/24/25  NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
