29 CFR  1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes:  a) 2104 E. Columbus Dr., Tampa, FL., - on or about 04/25/2025, employees in the process of installing air conditioning system materials were exposed to a 11'1" fall hazard, in that, floor holes utilized as access to the 2nd and 3rd floors were not covered or a guard rail system erected around.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) 2104 E. Columbus Dr., Tampa, FL., - on or about 04/25/2025, employees were exposed to a 11'1" fall hazard, in that, the extension ladder did not extend 3 foot above the landing surface.
