29 CFR 1910.28(b)(13)(i): The employer did not ensure that when work is performed less than 6 feet (1.6 m) from the roof edge, each employee is protected from falling by a guardrail system, safety net system, travel restraint system, or personal fall arrest system.  On or about May 2, 2025, an employee engaged in commercial electrical activities, located at 100 W Centennial Rd, Papillion, NE 68046, were exposed to fall hazards in that the employees were working in excess of 4-feet while conducting electrical  operations without fall protection.  It was documented that the employee was repeatedly reaching over the top and bending down to reach the lower portion of the canopy they were working from often exposing himself from the waist up.

29 CFR 1910.30(a)(1): The employer did not provide training for each employee who uses personal fall protection systems or who is required to be trained as specified elsewhere in this subpart before any employee is exposed to a fall hazard. And/or the employer did not ensure employees are trained in the requirements of this paragraph on or before May 17, 2017.  On or about May 2, 2025, an employee engaged in commercial electrical activities, located at 100 W Centennial Rd, Papillion, NE 68046, were exposed to fall hazards in that the employees were working in excess of 4-feet while conducting electrical  operations without having been trained in the uses of personal fall protection systems.
