29 CFR  1910.28(b)(1)(i):Except as provided elsewhere in this section 29 CFR 1910.28, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the systems described in 29 CFR 1910.28(b)(1)(i):  JOBSITE/306 DANBURY ROAD, UNIT 2, WILTON, CT 06897: On/or about May 5, 2025, the employer did not ensure that employees who were performing gutter cleaning work on a 4:12 pitch roof with an eave height of approximately 15 feet above the ground were provided with fall protection systems exposing one of the employees to serious injuries after falling from the roof.

And/or the employer did not ensure employees are trained in the requirements of this paragraph on or before May 17, 2017.  JOBSITE/306 DANBURY ROAD, UNIT 2, WILTON, CT 06897: On/or about May 5, 2025, the employer did not provide the employees with training on guard rails, safety net systems, and personal fall protection system/fall protection devices, fall hazard identification and the procedures to be followed to minimize such hazards.
