The in-plant handling, storage, and utilization of all compressed gases in cylinders, portable tanks, rail tankcars, or motor vehicle cargo tanks shall be in accordance with Compressed Gas Association Pamphlet P-1-1965, which is incorporated by reference as specified in Sec. 1910.6.  On or about May 6, 2025, at an establishment located at 338 South Main Street, Cedar Grove, Wisconsin, employees in the Torching Area the nitrogen compressed gas bottle (TC3AAM183) was unsecured, exposing employees to potential struck-by and projectile hazards.

29 CFR 1910.184(c)(10): Hands or fingers shall not be placed between the sling and its load while the sling is being tightened around the load.  On or about April 24, 2025, at an establishment located at 338 South Main Street, Cedar Grove, Wisconsin, an employee working in the Torching Area was using a hoist and alloy steel chain sling to hold a riser/gating for a casting and suffered an amputation injury when they placed their finger between the sling and the load causing their finger to be crushed.

29 CFR  1910.253(e)(5)(iii): Hose connections shall comply with the Standard Hose Connection Specifications, 1957, Compressed Gas Association.   On or about May 6, 2025, at an establishment located at 338 South Main Street, Cedar Grove, Wisconsin, the air hose used to supply abrasive powder to the Quality Company Powder Cutting Torch (Model: 4900) was attached to the hose fitting by a worm drive type hose clamp which was not designated for use on an air hose, thereby exposing the employees to the whipping action of the hose if the clamp were to come off.
