On or about May 6, 2025, at an establishment located at 338 South Main Street, Cedar Grove, Wisconsin, the air hose used to supply abrasive powder to the Quality Company Powder Cutting Torch (Model: 4900) was attached to the hose fitting by a worm drive type hose clamp which was not designated for use on an air hose, thereby exposing the employees to the whipping action of the hose if the clamp were to come off.
