29 CFR  1910.134(m)(1):  Records of medical evaluations required by 29 CFR 1910.134 were not retained and made available in accordance with 1910.1020:  a)  Rotating Machinery - On or about May 7, 2025, the employer was required to perform respirator medical evaluations, as employees utilized various types of tightfitting half face elastomeric respirators. There were also employees that are required to utilize loose fitting respirators, such as powered air purifying respirators and airline blasting hoods. The employer did not maintain employee respirator medical evaluations as required by this section.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.

29 CFR 1910.157(g)(2): The educational program to familiarize employees with the general principles of fire extinguisher use and the hazards involved with incipient stage fire fighting was not provided to all employees upon initial employment, and at least annually thereafter:  a) Rotating Machinery - On or about May 7, 2025, employees perform welding in the shop and utilize flammable liquids, such as but not limited to, mineral spirits and aerosol chemicals.  Employees were provided fire extinguishers to use during the incipient stage of fire fighting, and the employer did not provide annual education to familiarize employees with the general principles of fire extinguisher use and the hazards involved with incipient stage fire fighting.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.

29 CFR  1910.1026(d)(2)(iii):The employer used the scheduled monitoring option, and monitoring revealed employee exposures to chromium (VI) to be at or above the action level; however, the employer did not perform periodic monitoring at least every six months:    a) Rotating Machinery - On or about May 7, 2025, the employer had hexavalent chromium air monitoring conducted for welding operations on stainless steel.  An employee who was monitored on November 13, 2024, had a personal monitoring result of 4.6 µg/m³ for hexavalent chromium for an 8 hour time weighted average. These results exceeded the action level, and the employer did not conduct follow-up monitoring within 6 months.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.

29 CFR  1910.1026(h)(1):  A hazard was present or was likely to be present from skin or eye contact with chromium (VI); however, the employer did not provide appropriate personal protective clothing and equipment at no cost to employees, and/or did not ensure that employees used such clothing and equipment:    a)  Rotating Machinery - On or about May 7, 2025, the employer did not ensure that protective clothing (Company provided uniform) was utilized to protect employees from skin hazards associated with hexavalent chromium during stainless steel welding operations.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.
