29 CFR 1904.41(a)(1)(i):  Failure to submit record electronically - If your establishment had 20-249 employees at any time during the previous calendar year, and your establishment is classified in an industry listed in appendix A to subpart E of this part, then you must electronically submit information from OSHA Form 300A Summary of Work-Related Injuries and Illnesses to OSHA. You must submit the information once a year, no later than the date listed in paragraph (c) of this section of the year after the calendar year covered by the form.  a) 1500 Industrial Blvd. Kulpsville, PA - On or about May 8, 2025, the employer failed during year 2024, to electronically submit information from their OSHA form 300A by March 2, 2025.

29 CFR  1910.147(c)(6)(ii): The employer did not certify that periodic inspections of the energy control procedures had been performed. The certification shall include the machine or equipment which the procedure was being utilized, the date of the inspeciton:  a) 1500 Industrial Blvd. Kulpsville, PA - On or about May 8, 2025, the employer failed to certify that a periodic inspection was performed to ensure de-energization procedures were being properly implemented.
