29 CFR 1903.19(d)(1): The employer did not submit to the Agency documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required. (a) MG Quality Construction failed to document abatement within 10 calendar days after the abatement date that each of the following cited violations had been corrected. Citation 1, Item 2 a serious violation of 29 CFR 1926.503(a)(1) with an abatement date of June 11, 2025. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATIONS OF THE CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b). (a) On or about May 8, 2025, at 10470 Singing Lark Ln, Oak Creek, WI 53154, employees installing a roof on a residential home with a 10:12 pitch were not using fall protection. This exposed the employee to a fall hazard of approximately 17 feet 7 inches from the roof eve to the ground below on the back (east) side of the home.
